Introduction and Objectives
The final step in approach of gender reassignment therapy in female-to-male transsexuals consists

of the construction of a neophallus. Different techniques have been used to obtain rigidity in the neophallus, but often, they resulted in complications and failure. In this video we present a penile prosthesis implantation in female to male transsexual.

Methods
In this video, we have successfully inserted a malleable prosthesis using the Coloplast Genesis Flexible Rod penile implant in a 45 year old transsexual, who had a large bulky neophallus constructed from a fasciocutaneous microvascular free flaps about 9 years ago. A Dacron tubular graft 9 mm diameter with one end closed like a windsock is created using an abdominal aortic aneurism Dacron stent graft and is placed around the penile implant cylinder before insertion. The best surgical approach to gain access into the base of the penis was accurately defined preoperatively by a combined color Doppler ultrasound and MRI study.

Results
Operative time was 90 minutes. A prosthesis was inserted in patient without intraoperative and postoperative complications, with a follow up of 12 months. The patient returned to his regular activities on postoperative day ten.

Conclusions
Despite possible complications, implantation of penile prosthesis remains the best option for achieving the possibility of sexual intercourse in female-to-male transsexuals.
